I am not an enrolled Apple Health (Medicaid) provider. Under Washington state regulations (WAC 182-502-0160), clients enrolled in Apple Health/Medicaid who choose to receive private out-of-network therapy must execute the official HCA Agreement to Pay for Healthcare Services (Form 13-879) prior to receiving services.
